commences on 1 July 2005.3Information
required for returnsFor section 62(2) of the Act, the
following information mustbe included in a return for a
worker—(a)if the worker is not a registered
worker—(i)theworker’sname,birthdayandresidentialaddress;
and(ii)the type of
cleaning work performed by the workerfor the person
giving the return;(b)if the worker is a registered
worker—the number givento the worker in the register of
workers.4Classification levelThe
prescribed classification level for sections 73 and 75 ofthe
Act is ‘Cleaning Services Employee Grade 2’ under thefederal Cleaning Services Award 2010.5Percentage for levyThe
prescribed percentage for section 83 of the Act is 1.3%.Current as at 1 July 2015Page

rate for section 87(1) of the Act is 2% a monthor part of a
month.7Declaration of reciprocating State and
