titleThisregulationmaybecitedastheCremationsRegulation2003.2CommencementThis regulation
commences on the day section 19 of the Actcommences.2ALabelling ashes(1)For
the Act, section 11(1)(a), the following requirements areprescribed—(a)a
label must be securely attached to the exterior of eachcontainer in which the ashes are
stored;(b)the label must be legible;(c)the label must include the following
information—(i)the deceased’s name;(ii)if the person in
charge of the crematorium has beengiventhedeceased’sdateofdeath—thedateofdeath;(iii)the
cremation date;(iv)the name and
address of the crematorium;(v)ifthepersoninchargeofthecrematoriumhasallocatedanumberforthecremationforthepurpose of identifying it—the
number.(2)Also,ifthedeceased’sashesarestoredinmorethan1container, each label must—(a)be consecutively numbered; andCurrent as at 10 August 2012Page

Cremations Regulation 2003[s 3](b)statethetotalnumberofcontainersinwhichthedeceased’s ashes are stored.Example for subsection (2)—A deceased’s ashes are stored in 2
containers. The label for 1 containerincludes the
words ‘No. 1 of 2’ and the label for the other containerincludes the words ‘No. 2 of 2’.3Record of particulars for cremation to
be keptFor section 14(1)(a) of the Act, the
following particulars, totheextenttheyaregiventothepersoninchargeofacrematorium, are prescribed—(a)the deceased’s name;(b)the deceased’s age at death;(c)the date and place of the
death;(d)the deceased’s usual, or last known,
place of residence;(e)the cremation date;(f)thedatethepermissiontocrematewasissuedbythecoroner or independent doctor;(g)thefollowingparticularsabouthowtheashesweredisposed
of—(i)iftheashesweredealtwithinaccordancewithsection 11(1)(b) of the Act—(A)the details of the applicant’s
instructions; and(B)thedateandwayinwhichtheinstructionswere carried
out;(ii)iftheashesweredealtwithinaccordancewithsection 11(2) of the Act—(A)the date the notice under section
11(3) of theAct was given to the applicant; and(B)the date and place of burial.Page
