for the Attorney-General to grant a fiat to startaproceedingintheAttorney-General’snametoenforceorprotectpublicrightsmustcontainthefollowinginformation—(a)the
applicant’s name, address, telephone number and faxnumber;(b)the
reasons why the applicant can not obtain completerelief in the proceeding if the fiat is not
granted;(c)whether the applicant has pursued
other forms of relief,for example, mediation.(2)Theapplicationmustbeaccompaniedbythefollowingdocuments—(a)the proposed originating
process;(b)a certificate signed by counsel
stating that—(i)theproceedingisappropriatetobestartedintheAttorney-General’s name; and(ii)theoriginatingprocessisappropriatefortheAttorney-General’s fiat; and(iii)the applicant can
not obtain complete relief if thefiat is not
granted;(c)an opinion from counsel as to the
likelihood of successof the proceeding that details the
facts of the case andexplains the proceeding;Reprint 1Page

2](d)acertificatesignedbytheapplicant’ssolicitorstatingthattheapplicantisanappropriatepersontoactasrelator in the proceeding;(e)adocumentsignedbytheapplicantortheapplicant’ssolicitors
undertaking that—(i)theproceedingisatthesoleriskandcostoftheapplicant; and(ii)the
applicant will pay all of the Attorney-General’scosts
of the proceeding including any costs payableby the
Attorney-General to a party; and(iii)the
applicant indemnifies the Attorney-General forthe proceeding;
and(iv)the applicant will give the
Attorney-General copiesof all pleadings and other court
documents for theproceeding; and(v)theapplicantwillonlytakeanyofthefollowingsteps
with the Attorney-General’s approval—(A)amendtheoriginatingprocessfortheproceeding;(B)discontinue the proceeding or withdraw
partof it;(C)withdrawallorpartofanyanswertoacounterclaim;(D)settle all or part of the proceeding.(3)If a proceeding has already
started—(a)thissectionapplies,withnecessarychanges,toanapplication in
the proceeding; and(b)the application under this section
