s1 3 s4 Coroners Regulation 1998 CORONERS REGULATION 1998 [reprinted as in force on 4 December 1998] 1 Short title This regulation may be cited as the Coroners Regulation 1998 . 2 Commencement This regulation commences on 1 December 1998. 3 Additional things that must be done before permission and certificate may be signed—Act s 23A(2) (1) A person authorised to sign a permission and certificate must not sign the permission and certificate unless 1 (a) the person has made an external physical examination of the body of the deceased person; and (b) if the person is relying on a medical certificate mentioned in section 23A(2)(a)(i) of the Act—the person has seen a statement about the deceased person, in the approved form, from the medical practitioner who personally attended the deceased person. (2) The approved form must include provision for information to be given about whether the body of the deceased person contains or did contain a cardiac pacemaker or radioactive implant. 4 Particulars register must contain—Act, s 23F(3) The particulars the register must contain of each cremation at the crematorium are in the schedule. 1 See the Act, section 23A (Conditions of cremation) for further things that must be done before a certificate and permission to cremate may be signed.
4 Coroners Regulation 1998 SCHEDULE PARTICULARS OF CREMATION FOR REGISTER section 4 1. The following particulars of the deceased, to the extent the particulars are given to the officer in charge of the crematorium— (a) name; (b) age; (c) date and place of death; (d) usual or last known place of residence. 2. Cremation date. 3. Date of permission and certificate to cremate. 4. Any written directions, in the approved form, of the applicant for permission to cremate for dealing with the ashes remaining after cremation, that accompanied the permission and certificate to cremate. 5. The following particulars about how the ashes were dealt with— (a) if they were given to someone—the name of the person to whom, and the date on which, they were given; (b) if they were disposed of by interment by the officer in charge of the crematorium—the date and place of interment; (c) if they were dealt with in another way—the date and way they were dealt with. 6. In chronological order, the number of the cremation in the sequence of cremations conducted at the crematorium.
