application to clerk of the court forfresh
warrant(1)An approved form of an application to
the clerk of the courtBrisbane for the issue of a fresh
warrant under the Act, section5(3)1must provide for the following
information—(a)thenameoftheauthorisedpoliceofficermakingtheapplication;(b)the
type of warrant cancelled;(c)the purpose for
which the warrant was issued;(d)the
day and place the warrant was issued;(e)the
day the warrant was cancelled.(2)The
approved form must also provide for the application to besigned by the authorised police officer
making it.3Approved form of application to judge
for fresh warrant(1)An approved form of an application to
a judge of the Districtor Supreme Court for the issue of a
fresh warrant under theAct,section6(5)2mustprovideforthefollowinginformation—1Section 5 (Cancellation of warrants) of the
Act2Section 6 (Bench warrants) of the
Act

the Act, section 5(1), Police Headquarters, Brisbane, is aprescribed place.(2)For
the Act, section 6(2), the Office of the Sheriff, SupremeCourt, Brisbane, is a prescribed
